In order to facilitate our record search, please provide as much information as possible, as required below:

BAPTISMAL:

• Name of Baptised
• Date of Birth
• Date of Baptism
• Name of Parents 
• Name of Godparents
• Name of Baptism Minister (Priest)

CONFIRMATION:

• Same as for baptismal (see above)
• Date of Confirmation
• Name of Confirmation Minister (Priest)

MARRIAGE:

• Names of Groom and Bride
• Names of their parents
• Date of Marriage
• Name of Marriage Minister (Priest)

FUNERAL (DEATH):

• Name of Deceased
• Name of Parents
• Date of Birth
• Date of Death
• Date of Funeral

There is a $30.00 fee for each certificate.
